Patients First Visit

1. Completed paperwork (if it was mailed out to patient prior to appointment)
2. Insurance cards
3. Copay for office visit with a specialist
4. If insurance is through someone other than patient we will need insurance holder’s name, date of birth and employer.
5. Referral if required by insurance policy
6. List of Medications and Dosage amounts
7. Any Xrays, MRIs, or any other testing that has been done related to this problem. Also any previous doctor’s notes related to this problem.
8. If work related injury:
   Name and address of insurance company, date of injury, claim number, adjuster name and phone number.
9. If auto accident:
   Name and address of insurance company, date of injury, state injury, claim number, adjuster name and phone number.

You may want to start a list of questions for the Doctor and bring that to your first office visit as we sometimes have patients tell us that it makes it easier to remember everything they want to ask us.
